All participants must sign/confirm acceptance of these details. The details must be completed by a parent or guardian for any participant under 18 years old.
Please note that these activities involve some physical exertion.
You must not participate in this activity if any of the following applies to you, without first obtaining your doctors written approval which we will need to see before you take part
- Suffer from heart disease, high or low blood pressure or any cardiovascular problems unless satisfactorily controlled by appropriate medication/treatment.
- Undiagnosed pains in heart or chest, particularly associated with minimal effort.
- Breathing difficulties, including asthma where it is not satisfactorily controlled by medication.
- Epilepsy where this is not satisfactorily controlled by medication.
- Back pain or limited movement in any joint, particularly where this is made worse by exercise.
- You are recuperating from any serious illness, injury or operation.
- You are currently pregnant or have given birth and experience difficulties during pregnancy.
- You are taking any medication which may cause drowsiness.
- You have frequent episodes or feeling faint or spells of dizziness.
While every effort will be made to accommodate your needs, your safety is our first consideration and it may be necessary to review your involvement in activity.
SAFETY INFORMATION
If you have any concerns upon any of these matters, please discuss these with an instructor prior to the session.
For your own safety & comfort:
- Buoyancy aids must be worn at all times
- Helmets are required for certain activities. They must be worn at all times if given by instructor.
- Alcohol or recreational drugs have not been consumed before or during the event. Anyone found to be under the influence of drugs
or alcohol before partaking in the activity will not be allowed to take part. - Tie long hair back
- Remove jewellery and body piercings. (Please do not bring these to the session.)
- Please dress appropriately for all weather conditions. Suitable clothing must be worn.
- Bring all medication you may need throughout the activity.
- Wear sensible sports shoes. Open toed shoes must not be worn.
- Wear clothes you don’t mind getting dirty or even slightly damaged.
- Bring swimming costume and a towel.
- Cover all cuts and open wounds with waterproof dressing.
- Shower after activity. (except for 4×4)
- Please be aware that activities of a physical nature will occasionally result in contact with equipment or obstacles, which may give rise to bruises or other minor bumps or scrapes.AQUA PARK
- The Aqua Park runways and obstacles are anchored and floating on the water, as such may be slippery and unstable to walk on.
- Never, under any circumstances swim under the obstacles. Always go over the equipment.
- Never dive headfirst into the water from any part of the Aqua Park.
- Always look up when climbing. Do not climb underneath someone above you as people can fall when climbing.
- Make sure the water landing area is clear before jumping or sliding into the water.
- In using Ride Leisure’s equipment, I acknowledge and accept that there may be risk of injury to myself or my group.
- You agree to abide by rules above and as stated in the safety video and briefing.
